Image Select Field
Add a field to your contact forms allowing your visitors to visually select images from a list of options.
Jetpack Blocks require the Block Editor (also referred to as the Gutenberg editor). If you’ve installed the Classic Editor plugin, you have a few options:
- Deactivate the Classic Editor: Go to Plugins ? Installed Plugins in your WP Admin and deactivate the Classic Editor plugin.
- Enable both editors: If you prefer flexibility, navigate to Settings ? Writing in your WP Admin (not the menu under Jetpack settings). Set Allow users to switch editors to Yes. You will only see these options if you have the Classic Editor plugin installed and active.
Some custom themes or plugins may prevent you from using the Block Editor. In that case, please contact the support team for the plugin or theme for further assistance.

With the Image Select Field block, you can add images to your forms, allowing submitters to select their preferred image from a series of options.
Add an Image Select Field
To let visitors select images, add the Image Select Field:
- Open the page or post where you want to add your form, or create a new form. You can read how to do it on this page.
- In the List View, find the field that will come before or after the new upload field.
- Click the Options icon (?) in the toolbar, then choose Add before or Add after to insert a new block.
- Click the plus icon (+) block inserter and search for “Image Select Field”.
- Select Image Select Field from the block options to place it in your form – the icon is green.
Customize the Image Select Field
You can customize the default label (it says “Choose one image“) by clicking and changing the text.
You can also add an image by clicking on the placeholder image and then adding an image by uploading or from the Media Library.
Add a label to the image by clicking the “Add label” text below. Once you’re done, click Save or Publish in the top-right corner of the WordPress editor to keep your changes.
You can style and adjust the individual image fields the same way you customize any other images in the Editor.
Viewing Selected Images
When using the Image Select Field, you can see the image that the user selected by viewing the form response at Jetpack -> Forms -> Responses

Privacy Information
The Image Select Field is activated by default. It can be deactivated/reactivated at any time by following the guide to control Jetpack features on one page.
